The LifeHelp Nutrition and Diabetes Center at St. Anthony's Hospital, offers a variety of nutrition and lifestyle education services to help you live a healthier, happier life. Our staff of registered dietitians, registered nurses, exercise physiologists and certified diabetes counselors, work with you and your physician to develop a personalized wellness plan. Our goal is to provide the tools and information you need to take charge of your health and nutrition.

To Enroll in a LifeHelp Program
Please call our office at (727) 820-7910. A written referral from your physician may be required.
Physician Referral Forms
If the LifeHelp Center Program that you would like to enroll in requires a referral, you may print this referral form and take it to your physician.
- If you are a physician, please print the referral form, fill it out and refer your patient to our services.
Insurance
Outpatient diabetes education programs are covered by most insurance plans. Authorizations are required for managed care plans.
