Please have the following information ready when you call to schedule your appointment:
- Patient's name
- Date of birth
- Home and cell phone number
- Insurance card/information
- Exam type/presciption
- Referring physician's name
Preparing For Your Appointment
You will receive specific instructions on how to prepare for your exam at the time you schedule your appointment. You may also receive a follow-up call from a certified technologist the day prior to your exam to review preparation instructions. Please arrive 20 minutes prior to your scheduled appointment time.
Please bring with you a picture ID, your insurance card and other pertinent information.
Rescheduling Your Appointment
We do recognize emergencies can arise that may make notifying us prior to your appointment impossible. We hope you will reschedule your appointment for another day by contacting one of the numbers above.
Request for Images/Medical Reports
Your Images/Medical Reports are available on CD or film for pick-up at any of our BayCare Imaging Service locations. Please allow 24 business hours for delivery to the imaging location where you will be picking up your CD or films.

St. Anthony's and BayCare Health System is a preferred provider with most major health insurance providers. Your insurance coverage is based on criteria outlined in your insurance plan coverage document. It is best to check with your insruance company if you are unsure about whether they will cover the test. Even if your insruance plan covers the test, you will be responsible for any co-payment, co-insurance, or deductible amount specified by your insurance plan.
If you do not have insurance coverage, ask about our self-pay rates when you schedule your appointment.
For questions about your bill for medical imaging services provided at a BayCare Imaging Services facility or to set up a payment plan, please contact our central billing office at (727) 820-7676.
If you were seen at one of our Outpatient Facilities, you will receive one bill for your procedure. If you were seen at St. Anthony's Hospital Radiology department, you will receive two bills: one from the facility where you had your exam and one from the Radiologist Group for the interpretation of the procedure. It is possible that you did not see the radiologist at the time of your exam. However, once the studies were made, the radiologist interpreted the studies, dictated the results and a copy was sent to your attending and/or referring physicians. When needed, the radiologist also consults personally with your physician.
